Brave Heart NP In Psychiatry, PLLC policies were created to support, consistent, thoughtful care and to set clear expectations from the start. Brave Heart NP In Psychiatry, PLLC is intentionally created to provide exceptional, empathetic, compassionate care for those faced with life’s most difficult challenges.
Practice Scope and Safety Boundaries
Brave Heart NP In Pshychiatry, PLLC is an outpatient psychiatric practice designed for individuals struggling with anxiety, depression, grief and trauma which provides holistic integrative care for adults located in New York State
Care is intentionally designed for clinical expertise—integrating psychiatric evaluation, medication management when appropriate, therapeutic support, education, and aligned with those struggling with catastrophic illness/cancer survivorship.
Brave Heart NP In Psychiatry, PLLC has limited office hours and is not structured to support crisis interventions or rapid access care. If your needs require urgent support, a higher level of care, or frequent contact, Brave Heart NP In Psychiatry may recommend a higher level of care and support (for ie: intensive outpatient programs, higher-frequency psychotherapy, or specialized services).
Brave Heart NP In Psychiatry, PLLC does not provide crisis interventions, emergency services, or on-call coverage. If you are experiencing a crisis, feel unsafe, or need urgent support, call 988, call/text 988, or go to the nearest emergency department.

Cancellations and Missed Appointments
Brave Heart NP In Psychiatry,PLLC was designed as a smaller private practice to provide greater access to longer sessions and highly compassionate, exceptional care. Missed appointments and late cancellations adversely impact clinical continuity causing a reduction of availability for other patients.
Cancellations Require 48 Hours prior Notification/ Fees: All appointments at least 48 hours’ notice to cancel or reschedule an appointment. Cancelled appointments outside the notice window, as well as missed appointments, are billed at a rate of $150 for 30-minute sessions and $225 for an initial comprehensive psychiatric evaluation.
Canceling/rescheduling: To cancel or reschedule, please contact Brave Heart NP In Psychiatry, PLLC as directed in your appointment confirmation message. Cancellations are considered effective once confirmed.
Once appointments are scheduled, patients are responsible for tracking appointment dates and times
Credit cards on file will be used for applicable late-cancellation or missed appointment fees.
Exceptions: Illness or unexpected emergencies are considered exceptions and may be considered at the clinician’s discretion.
Medication and Refill Policy*
Brave Heart NP In Psychiatry, PLLC medication treatment strategies are made thoughtfully guided by evidence-based studies and careful assessment. If medication is part of care, adjustments are made over time based on patient response to treatment while taking into consideration benefits, potential side effects, and goals.
Prescribing is provided only when you are located in New York State at the time of the appointment.
Refills are part of ongoing care and are ordered within scheduled appointments and are prescribed for the duration until the next appointment. Patient response is based on clinical monitoring and treatment plan.
Brave Heart NP In Psychiatry does not provide one-time prescriptions or refills for medications that were not originally prescribed within the practice. Controlled medications are part of care that necessitates additional monitoring and structured refill timelines apply.
Refill Request Timing: Plan refill requests well in advance of travel, weekends, holidays. All refill requests require at least 3 business days in advance. This will ensure refills can be handled during regular business hours. All refills are typically reviewed during follow-up visits, and Brave Heart NP In Psychiatry, PLLC does not provide after-hours urgent refills.
Refills are ordered during scheduled sessions and for the duration until the next appointment, based upon patient response to medication management strategies and treatment plan.
Plan ahead for any potential lapses in medication coverage: If you are out of states or out of the country—including upcoming travel, school schedules, holidays, or pharmacy access issues—please request refills ahead of time. Last-minute requests may not be possible to accommodate.
Examples
If you will run out Monday, request the refill by Wednesday of the prior week.
If you will run out Friday, request the refill by Monday.
If you anticipate travel, school schedules, time away, being out of state, or leaving the country, request refills 1–2 weeks in advance if possible to allow for pharmacy processing and coordination.
Business hours only: Refills are handled during regular business hours. Brave Heart NP In Psychiatry does not provide after-hours, weekend, or urgent refills.
*Prior Authorizations and Pharmacy Availability:
Many medications require insurance review, prior authorization, or may be intermittently out of stock. Brave Heart NP In Psychiatry, PLLC will complete prior authorizations during regular business hours. Approval timelines are solely determined by your insurer and are not guaranteed.
Pharmacy inventory are outside the practice’s control. Patients are responsible for selecting a pharmacy and confirming medication availability. If medication is unavailable, please notify Brave Heart NP In Psychiatry, PLLC with your preferred alternative pharmacy
Brave Heart NP In Psychiatry, PLLC will not call multiple pharmacies to locate inventory. Multiple pharmacy transfers and same-day changes may require an appointment and cannot be accommodated urgently.
Brave Heart NP In Psychiatry, PLLC does not provide change of pharmacy on a monthly or refill basis.
*Pharmacy and Continuity:
To ensure patient safety, Brave Heart NP In Pshychiatry, PLLC prescribes medications only within an established treatment relationship and with clinical follow-up appointments. Brave Heart NP In Pshychiatry, PLLC does not provide one-time refills for medications or prescriptions or that were not originally prescribed within this practice.
Requests related to misplaced, lost, stolen, damaged, “running out early “require an appointment and additional clinical review.
Replacement prescriptions cannot be guaranteed and may be declined when they cannot be supported safely or clinically.
Brave Heart NP In Psychiatry, PLLC cannot provide “bridge” prescriptions for new clients outside of an established care relationship. When appropriate, Brave Heart NP In Psychiatry, PLLC may recommend consolidation to a single pharmacy to safety risks, reduce duplication and errors.
Controlled Medication
When controlled medications are part of a regimen, refill timing and monitoring are more intensive and will be reviewed extensively during appointments.
Controlled medication refills are ordered within scheduled appointments and are not handled urgently.
*Medication Safety and Monitoring:
Prescribing medication requires ongoing clinical monitoring. When prescribing high risk medications—Brave Heart NP In Psychiatry, PLLC requires additional safety steps such as more frequent appointments, labs, coordination with other clinicians, review of medical records, and other monitoring measures appropriate to the medication.
Brave Heart NP In Psychiatry, PLLC reserves the right to adjust, pause, taper, or discontinue medications to ensure clinical safety and uphold treatment integrity practices.
Brave Heart NP In Psychiatry, PLLC is not able to accommodate early refills, urgent refills, or requests for controlled medications as a primary focus of care.
Medications must be taken only as prescribed. If there are concerns about misuse, unsafe use, or escalating risk, abuse or diversion, Brave Heart NP In Psychiatry, PLLC may require more frequent visits, coordination of care, additional monitoring, or changes to the treatment plan—including discontinuation of medication, or tapering —when clinically indicated.
Brave Heart NP In Psychiatry, PLLC may also recommend a higher level of care to ensure patient safety if clinically indicated.
Communication and Response Times
Business hours: Brave Heart NP In Psychiatry, PLLC responds during regular business hours on Monday-Fridays, 10:00 AM–7:00 PM (ET). Limited weekend appointments are available upon request.

If privacy cannot be established/maintained (e.g., driving, public spaces, others in the room), the session may be paused, terminated or rescheduled.
Patients are responsible for ensuring they have a reliable connection and a private setting for sessions.

Records, Forms, and Administrative Requests
Brave Heart NP In Psychiatry, PLLC will consider appropriate medical record requests, coordination of care letters, treatment summaries, and limited administrative paperwork within the scope of care. Requests are assessed case-by-case based on clinical appropriateness, practice capacity, and time required.
All administrative requests are handled during business hours and will require review of records and if necessary during scheduled appointments. These requests may require an administrative fee based on time required and complexity.
To protect privacy, clinical integrity, and the therapeutic relationship, Brave Heart NP In Psychiatry, PLLC will not provide active or retroactive medical or legal documentation (ie- ESA letters, custody/divorce statements, FMLA/disability paperwork, sick notes, or court-related documentation/medical-legal evaluations.). Requests falling outside the scope of treatment will be declined. Clinically appropriate medical record requests and routine paperwork directly related to treatment continuity will be provided with patient consent.
Financial Policy
Self-pay model: Brave Heart NP In Psychiatry, PLLC does not accept any insurance and is considered a self-pay practice. This allows for longer visits, greater access and clinical flexibility. Care is individually tailored to your needs rather than insurance-based requirements. Patients are responsible for payment at the time of service. Patients are responsible for reviewing practice fees and financial policies in advance.
Payment and Balances
Payment is due at the time of service rendered. Brave Heart NP In Psychiatry, PLLC will not carry ongoing balances. If a balance remains after a session due to a declined credit card, we will promptly reach out to resolve any payment issues before the next scheduled appointment.Out-of-Network Responsibility
Patients must verify confirm eligibility, benefits, and reimbursement terms directly with their insurance carrier to determine out-of-network reimbursement. Brave Heart NP In Psychiatry, PLLC will not communicate with insurers on patient behalf, verify eligibility benefits, or assume responsibility for reimbursement outcomes, insurance determinations, denied claims.
Card on file: A credit card on file is required to schedule appointments and will be used for session fees and late-cancellation without prior 48 hours notification or missed-appointment fees commensurate with the policies on this page.

Additional Clinical Services
Additional requests for extended record review, treatment summaries, forms, letters, or other non-routine requests, will incur a separate fee which will be discussed in advance when applicable.
